Topic: Appendicitis: approaching the inflamed appendix
By Omar Makhamreh MD

Abstract: Appendicitis is one of the most common abdominal emergencies requiring surgical intervention. It involves the inflammation of the appendix, typically caused by an obstruction that leads to bacterial overgrowth, localized infection, and, if left untreated, the risk of rupture and peritonitis. The clinical presentation often includes right lower quadrant abdominal pain, nausea, vomiting, and fever, though symptoms can vary, especially in atypical populations like children, the elderly, and pregnant women. Diagnosis is primarily clinical but is supported by imaging techniques such as ultrasound and CT scans. Early diagnosis and prompt surgical intervention, usually in the form of appendectomy, are critical in preventing life-threatening complications such as perforation. Surgical approaches include open appendectomy and the increasingly preferred laparoscopic appendectomy, which offers benefits like smaller incisions and quicker recovery. Postoperative care focuses on infection prevention, pain management, and monitoring for complications. This review provides an overview of the pathophysiology, diagnostic methods, and treatment strategies for appendicitis, highlighting the importance of timely intervention to reduce morbidity and mortality.
