Abstract:

Parapneumonic effusions (PPE) in children are a common complication of bacterial pneumonia, characterized by the accumulation of fluid in the pleural space. They can present as uncomplicated, complicated, or empyematous effusions, with varying degrees of severity and treatment requirements. The pathophysiology involves inflammation of the pleura due to infection, leading to fluid accumulation. Clinical presentation may include fever, pleuritic chest pain, cough, and difficulty breathing, although symptoms can be subtle in younger children. Diagnosis relies on imaging techniques such as chest X-ray or ultrasound, while thoracentesis and pleural fluid analysis help differentiate between the various types of effusions. Management strategies depend on the classification of the effusion, ranging from antibiotics and observation for uncomplicated cases to drainage procedures for complicated or empyematous effusions. Early recognition and intervention are crucial in preventing long-term sequelae such as lung scarring or respiratory compromise. This review discusses the epidemiology, pathogenesis, diagnostic approach, and management strategies for parapneumonic effusions in the pediatric population, emphasizing the importance of timely and appropriate care to optimize outcomes.
